Parents will be required to complete a registration pack prior to their child starting the club.  This will enable the team to always have the relevant information on all of the children in our care.  Children will not be allowed to stay at the club if we do not have a completed registration form.  We will ask you to update this form at regular intervals to ensure the most accurate information.

Booking forms can be downloaded with the club booklets sent to QEST. As soon as we have processed your booking and confirmed availability at that particular club we will send you confirmation that the places have been arranged for your child and the date of commencement.
